The Fareham & Portchester PCN is a network of four local practices who are working together to share resources, skills & experience to provide cost-effect, patient-centered health care for all patients in the Fareham and Portchester area.

Because of the PCN we can offer Enhanced Access Appointments. This enables us to provide additional appointment capacity across the four practices (called “Enhanced Access”), including GP, ANP, Practice Nurse, Musculoskeletal (known as First Contact Practitioners), Social Prescribers, Paramedics, Pharmacists, Pharmacy Technicians, and Mental Health & Wellbeing.

These are both telephone and face to face appointments that will be available on the weekend. Face to face appointments will be at Westlands Medical Centre.

Clinicians consulting with our patients will have read-write access to the patient record, and are able to make referrals and prescribe in the same way clinicians at the practice do.
